Our Hub aims to support locals with disabilities with support coordination, NDIS Access Support and  Psychology with more services to come in the near future.

Aimee – NDIS Coordinator

Aimee has a Bachelor of Arts in Psychology and while studying she worked as a support worker for six years, providing direct care to NDIS participants, youth, mental health, offender reintegration and out of home care. After this, Aimee moved into providing Support Coordination in the Sale and surrounds area. She has also assisted with supporting people to meet NDIS access and obtain NDIS plans. Aimee has a passion for providing fantastic service delivery and provides herself with listening to participants and empowering them to make their own choices and work alongside her to have control of their NDIS plans. In Aimee’s personal time, she likes to walk, read, watch time pieces & true crime and spend time with her family.

Services provided by Aimee:

  • Helping participants understand their NDIS plan.
  • Working with participants to plan and coordinate their supports.
  • Connecting participants with supports and services.
  • Establishing and maintaining supports.
  • Setting up service agreements and service bookings.
  • Understanding what support providers can and cannot charge for.
  • Anticipating barriers to accessing support and creating plans to tackle these.
  • Coaching participants on accessing supports.
  • Providing the NDIA with reports.
  • Coordinating with participants, their support workers and their family members to help participants exercise choice over their supports.
  • Supporting participants in finding ways to be more independent.
  • Planning for unexpected events, such as moving homes, changes in family support levels and unexpected hospital visits.
  • Creating plans to help participants avoid and/or mitigate crisis situations.
  • Submitting requests for plan reassessments.
